Middlesex County roads include Route 501, Route 514, Route 516, Route 520, Route 522, Route 527, Route 529, Route 531 and Route 535. Middlesex County hosts many county routes, state routes, U.S. routes, interstate and toll roads in the world. U.S. routes are: U.S. Route 1, U.S. Route 9, U.S. Route 1/9 and U.S. Route 130 and state routes: Route 18, Route 26, Route 27, Route 28, Route 32, Route 33, Route 34, Route 35, Route 91 , Route 171, Route 172 Route 184 and Route 440. To start near the intersection of Ridge Road & Route 1 in South Brunswick Township to exchange 8A in Monroe Township, The Turnpike Authority planned to build Route 92.The main rail lines that serve Middlesex are: North Jersey Coast Line, Northeast Corridor Line, and Raritan Valley Line. The North Jersey Coast Line travels through the eastern part of the county. The Northeast Corridor Line runs through the northern and central part of the county. MCDOT is dedicated to the development of a better combine passenger transportation network, particularly to achieving the goal of a seamless system where the County passenger transportation services allow connections to the local and regional bus and rail network in Middlesex County.

A seven-member Board of Chosen Freeholders is governed Middlesex County. County departments who oversee the various committees, chairpersons and members to the Director appoint Freeholders.
